It just a helicopter that fly's across the north eastern bridge. A lot like GTA IV's, but for SA. Known stuff: It spawns at a very high elevation, because if it starts at ground level, CJ wouldn't be near it, causing it to fall into water (over and over, may causing SA to crash due to a stack overflow).
